Completing the IELTS Application Form

1. Preferred Date of Test:
Enter your preferred test date. Also include a second choice. A list of dates appears on the IELTS Test Date Sheet.
2. Last (family/surname) name:
PRINT your legal family name (last name /surname) in the space available. (This name must be the same as the name on your passport)
3. Title:
CIRCLE the appropriate title or write it in the space provided in the application.
4. First (given) Names:
PRINT your first name and second name(s) in full. (These names must be the same as the names on your passport/National Identity Card and must appear in the same order.)
5. Address:
PRINT your street number, street name, apartment/unit number, city, province/state and postal code. If there is a buzzer number at your apartment building, please include this as well. You will only be sent onecopy of your result so please be sure the correct mailing address is given, since your results will be sent to this address.
6. Telephone/Mobile Number
Include a telephone number where you can be reached during the day.
7. Email:
Please provide an email address. Please note that our primary means of delivering information is via email.
8. Date of Birth:
Indicate your date of birth DD/MM/YY (please ensure that the year is correct).
9. Gender: Please mark Male or Female
CIRCLE the appropriate gender.
10. Passport or National Identity Card Number:
Evidence of identity is required. Candidates taking the test outside of their own country must present a current passport. Candidates intending to use the test to go overseas should also present the passport (US citizens). Passport must be valid. Enter the number of your passport in the space available. (Please be aware that you MUST include a copy of your passport with your completed application form and bring your passport on the day of your test.) The document must be valid/not expired on the test day.
11. Country or region of origin (code):
PRINT your country or region of origin code. The code is found in the application.
Country or region of origin (name):
PRINT your country or region of origin name which matches the code used. If your country or region of origin is not listed, enter 000.
12. First Language code/name:
PRINT your first language in the space available. Enter the code in the appropriate boxes. (Codes are provided in the application.)
13. Occupation sector/level:
Look for the closest description of your occupation and level. If your work and position is not covered at all, enter 00. Enter the code in the appropriate boxes. (Codes can be found as above.)
14. Why are you taking this test?
Select the correct number code and enter it in the appropriate box. (Codes can be found on page iv of the application.)
15. Which country are you applying to/intending to go to? (tick one box only)
Choose the country where you are intending to use your Test results. If other, please specify.
16. Which IELTS test format (module) are you taking? (tick one box only)

The Academic Reading and Writing tests assess whether a candidate is ready to study or train in the medium of English at an undergraduate or postgraduate level. Admission to undergraduate and postgraduate courses should be based on the results of Academic modules.
The General Training Reading and Writing tests are not designed to test the full range of formal language skills required for academic purposes. The emphasis of General Training is on basic survival skills in a broad social and educational context. This module is suitable for candidates who are going to English-speaking countries to complete their secondary education, to undertake work experience or training programs not at a degree level, or for immigration purposes. Listening and Speaking tests are the same for Academic and General Training modules.

At this time at inlingua®, we currently only offer the paper-based test. All parts of the test will be paper-based.

Describe your need and provide the hard-copy evidence to the test center. Please note that 3 months’ notice is required for these needs to be accommodated.
23. Name of College/University/Other Institution you would like results sent to:
PRINT the name and the complete address of the college/university/other institution you would like your test results sent to. (Please note: you may request information sent to up to five institutions at no charge for one month after the test date. For additional institutions or after one month, please include the processing fee of $10.00 per Test Report Form.)
Final steps
Attach a current passport-size photograph in the space provided at the top of the Application Form. Please do not attach a photocopy or digitally enhanced photograph (this might be sent to us via e-mail).
Sign and date the Application Form.
Include a copy of your passport together with your application. This is extremely important! Your application will ONLY be processed after we have a copy of your passport.

A. Credit cards: Visa, Master, American Express, or Discover.
B. Other: money order or cashier/certified check in US funds. Discover.
C. Online: via PayPal@inlinguaEnglish@edu.
